Spa Party / Group Contract Agreement & Cancellation Policy For parties of 4 or more with 2 or more services each

Thank you for choosing Dolce Organic Salon, Med Spa & Salt Room for your special event. We look forward to helping ensure your visit is as relaxing as possible.

Please review and complete the following forms which will guide you through the booking process, while providing us with all the information needed to make your day extraordinary. Please review the contract carefully and complete all sections (as applicable).

Please review the contract carefully. Upon completion, return the Agreement via email to
info@dolceorganicsalon.com, drop it off at the spa, or mail it to the address listed above. Once received, the requested date and times for your event will be checked for availability and reserved if available. Please understand that we are unable to reserve appointments until the completed Spa Party /Group Contract Agreement is received.

Once this form is received, we will work on accommodating your requests. Within 72 hours, we will email you a proposed schedule. You will then have 48 hours to respond with confirmation and /or changes. Once you confirm the schedule, a $200 deposit will be required to finalize the appointments. If we do not hear back from you within 48 hours, the time slots reserved for you will be released.
